In participating in the services offered at Pilates by Jen, I agree that I do so at my own risk. In consideration of being permitted to use the facilities and equipment located at Pilates by Jen, I understand that Pilates by Jen shall not be liable for the loss or theft of, or damage to, the personal property of a guest or member.
I assume full responsibility for any injuries or damages which may occur using studio facilities and equipment, and I hereby fully and forever release and discharge Pilates by Jen, its owners, and instructors from any and all claims, demands, damages, rights of action, or causes of actions, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting or arising out of my or a member's use or intended use of the facilities and equipment, on or about the premises of Pilates by Jen.
I acknowledge that I am fully liable for all medical expenses as a result of any injury incurred during my participation in the services offered at Pilates by Jen.
I warrant, represent, and agree that I am in good physical condition and have no disability, impairment, or ailment preventing me from engaging in active or passive exercise that will be detrimental to health, safety, comfort, or physical condition if I do so engage or participate. If my health condition changes, I acknowledge that it is my obligation to take full responsibility for my health, wellbeing, and safety during any type of exercise, and avoid any exercises I have been advised by my physician not to do.
Consent: I understand and acknowledge that I have the right to consensual practice and that consent is a requirement of tactile feedback and physical touch for all services at Pilates by Jen. I understand that I will be given the option in every class to anonymously give consent for hands-on adjustments.
Minors Policy: I understand that all minors under the age of 18 must be accompanied by an adult at all times at Pilates by Jen, and a liability waiver must be signed by the minor's guardian or caregiver on behalf of the minor.
COVID-19 Warning: I acknowledge the contagious nature of the Coronavirus/COVID-19. I acknowledge that Pilates by Jen has put in place preventative measures to reduce the spread, and it is a condition of entry to comply with the Pilates by Jen COVID-19 Health and Safety Plan. I understand that Pilates by Jen cannot guarantee that I will not become infected with the Coronavirus/COVID-19 and I voluntarily seek the services provided.
